Role Description Registered Nurse (RN) role involves supervising and guiding Care Professionals, ensuring adherence to care plans, and participating in quality improvement activities. This is a part‑time hybrid role based in Jacksonville, FL, with a combination of field visits and some work‑from‑home tasks such as documentation, care coordination, and telephonic follow‑up. Qualifications Active, unencumbered Registered Nurse (RN) license in the state of Florida and current CPR/First Aid certification. Experience in home health, geriatric care, or community‑based nursing, with strong skills in patient assessment, care planning, and case management. Strong communication and interpersonal skills, including the ability to educate clients and families, collaborate with multidisciplinary teams, and build trust with diverse populations. Solid documentation and organizational skills, with comfort using electronic health records or digital charting tools and managing multiple cases effectively. Commitment to person‑centered care, safety practices (including fall prevention and infection control), and adherence to AHCA and agency compliance standards. Valid driver’s license, reliable transportation for home visits in Northeast Florida, and the ability to work in a hybrid model (field and remote tasks).
