Pay Rates – Per Completed VisitPer-Visit - $42
Downtime (pre-approved wait time) - $30/hr
"Completed" = visit done in the home + documentation finished in EMR within 24 hours + orders/signatures uploaded + meets QA standards.
Service Area & ScheduleCleveland / Northeast Ohio | Weekdays, weekends, evenings, on-demand - with a flexible schedule.
What You'll Do- Provide direct patient care to palliative care patients in their homes in accordance with the established plan of care
- Monitor and document patient symptoms, comfort levels, and changes in condition
- Administer medications and treatments as directed by the RN Case Manager or physician
- Communicate effectively with patients, families, and the interdisciplinary care team regarding patient status and care goals
- Provide education and emotional support to patients and families managing serious illness
- Maintain accurate and timely documentation in the EMR within required timeframes

- Mileage reimbursement at the IRS standard rate ($0.70/mile)

- Responsive support team - scheduling, intake, clinical supervisors, triage, and after-hours coverage so you're never on an island
- Public Service Loan Forgiveness (PSLF) eligible employer - qualifying employees may be eligible for federal student loan forgiveness after 120 payments
- One of the only home health agencies in Northeast Ohio with a dedicated Behavioral Health nursing program
Must-Have:
- Active LPN license (Ohio) + current BLS certification
- Reliable vehicle and valid driver's license with ability to travel throughout the Cleveland / Northeast Ohio service area
- Minimum 1 year of clinical nursing experience
Preferred / Nice-to-Have:
- Prior palliative care, hospice, or home health experience
- Experience supporting patients and families through serious illness
- Familiarity with HCHB or similar home health EMR
- Strong communication and family support skills
