Dialysis Social Worker & Patient Advocate
Concerto Renal Services
Concerto Renal Services is seeking a Social Worker to join its team. This role provides psychosocial evaluations, counseling, and quality‑of‑life support for dialysis patients in Skilled Nursing or rehab facilities. You will address psychosocial needs and maintain detailed documentation to meet CMS guidelines.
