Trans Housing Case Manager II - Advocacy & Access
Community Forward SF
Community Forward SF is hiring a Case Manager II for the Ending Trans Homelessness initiative in San Francisco. You will manage a caseload, deliver individualized service plans, connect participants to benefits, housing resources, and eviction-prevention services, and coordinate with Housing Specialists to place clients in stable housing.
