Home Health Nurse

We are seeking a compassionate and adaptable Registered Nurse (RN) to provide high-quality care in a Home Health and Hospice setting across the Grand Rapids Area. This role is ideal for a nurse who thrives in dynamic environments, values patient-centered care, and is comfortable working independently in patients’ homes. Job Description Job Title: RN Home Health Location: Grand Rapids, MI Schedule: 5x8 hrs Shift: Days Pay Range: $2200 - $2463 weekly gross DOE Key Responsibilities Deliver skilled nursing care to home health and hospice patients in accordance with individualized care plans Perform comprehensive patient assessments, including physical, emotional, and psychosocial needs Administer medications, treatments, and interventions as prescribed Monitor patient progress and adjust care plans in collaboration with the interdisciplinary team Provide education and emotional support to patients and their families/caregivers Maintain accurate and timely documentation in compliance with regulatory standards Coordinate care with physicians, therapists, social workers, and other healthcare professionals Participate in on-call rotation and respond to patient needs as required Travel between patient homes; mileage reimbursed between visits (first to last patient) Requirements Current RN state license 2+ years of Home Health experience 1–2 years RN clinical care experience OASIS experience Homecare Homebase (HCHB) experience Current CPR certification Current driver's license Reliable transportation + valid insurance Graduate of an accredited ADN or Nursing Diploma program Ability to travel locally and work independently Additional Details Weekly Hours Guarantee: 40 hours Note: We also have other openings available if you would like to explore.
