About the Position

TravelNurseSource is working with United Health Care Staffing, Inc. to find a qualified Utilization Review RN in Portales, New Mexico, 88123!

RN Case Manager – Utilization Review & Swing Bed Coordinator for a 13-week contract assignment. The RN will be responsible for utilization review, discharge planning, swing bed coordination, and case management services. Responsibilities include evaluating and screening potential swing bed admissions, coordinating patient transitions of care, communicating with patients, families, physicians, and interdisciplinary team members to facilitate timely discharges, managing referrals and prior authorizations, completing required documentation, assisting with MDS documentation as needed, educating providers and staff on appropriate level of care and documentation requirements, and performing utilization reviews using InterQual criteria for inpatient and observation admissions. Documentation is completed in the Meditech EMR

Requirements

• Minimum 5 years of recent RN Case Management experience
• Active RN license
• Experience in Utilization Review
• Experience in Discharge Planning
• Experience coordinating Swing Bed admissions and patient placement
• Knowledge of Medicare, Medicaid, HMO, Swing Bed, and private insurance coverage requirements
• Experience with InterQual utilization review criteria
• Experience obtaining referrals and prior authorizations for:
  • Home Health
  • Skilled Nursing Facility (SNF)
  • Acute Rehabilitation
  • Durable Medical Equipment (DME)
  • Follow-up appointments
• Experience with MDS documentation preferred
• Meditech EMR experience preferred
• Strong communication, care coordination, and interdisciplinary collaboration skills
• Ability to work Monday–Friday, 8:00 AM–4:30 PM
• No holiday or on-call requirements
