Clinical - LTSS Service Care Manager

Immediate need for a talented Clinical - LTSS Service Care Manager . This is a 06+ Months Contract opportunity with long-term potential and is located in Winston, NC(Hybrid) . Key Responsibilities: Evaluate member needs and available resources, recommending plans for optimal outcomes. Develop and monitor long-term care/service plans, identifying necessary providers and community resources. Facilitate communication between members, families, and care provider teams to ensure accessibility to services. Provide resource support for various needs, including employment, housing, and independent living. Conduct ongoing assessments and document care management activities to ensure compliance with regulations. Perform home and site visits to assess member needs and collaborate with healthcare providers. Educate members and families on healthcare procedures, service options, and benefits. Offer feedback to leadership on opportunities to enhance care quality and service delivery. Key Requirements and Technology Experience: Bachelor's degree in a human services field (psychology, social work, nursing, etc.) or equivalent experience. 2 - 4 years of related experience in care management or service delivery. RN or LCSW license required for specific states. Experience in LTSS and/or HCBS coordination and care management is preferred. Experience in social work, geriatrics, gerontology, pediatrics, or human services. LCSW-A preferred for North Carolina Tailored Plan.
