Seven Healthcare are seeking a skilled Physical Therapist for a contract assignment in Carmel, IN. This role specializes in providing home health services, evaluating and treating patients based on physician referrals. The therapist will be working within a supportive environment, focusing on enhancing patient independence and function.

- Perform physical therapy evaluation and treatment of patients based on physician orders/referrals.
- Complete comprehensive evaluations using appropriate tools and procedures.
- Review patient records for diagnosis, history, precautions, and contraindications.
- Establish measurable goals and develop treatment plans to maximize patient independence.
- Regularly reassess treatment effectiveness and modify goals or treatments as needed.
- Communicate patient needs and progress to the treatment team and involved parties.
- Consult with medical providers regarding patient evaluation, treatment, and discharge.
- Recommend and assist in securing adaptive equipment to enhance rehabilitation programs.
- Maintain and submit required documentation for evaluations, treatments, and billing.
- Adhere to professional practice standards and applicable laws.
- Supervise Physical Therapist Assistants and Aides in direct patient care.
- Participate in internal staff development programs and meetings.
- PT license in Indiana
- Experience in home health and proficiency with Home Care Home Base
