Certified Nursing Assistant (CNA) - Long Term Care (LTC)

Supports residents in skilled nursing and long-term care facilities with daily living activities and routine medical support under supervision of licensed nursing staff. The role emphasizes ongoing patient comfort and safety in a residential setting.

Works to prevent complications from immobility, maintain dignity, and communicate changes in resident condition to the care team.

Responsibilities

• Assist with ADLs and routine grooming, feeding, and toileting for residents.
• Monitor and report residents' vital signs and any changes to nursing staff.
• Provide repositioning, turning, and skin care to reduce risk of pressure injuries.
• Support mobility and transfers with proper equipment; encourage participation in activities as appropriate.
• Maintain clean, safe resident rooms and common areas; respond to call lights promptly.
• Document care provided and update care plans as directed by supervisors.
• Support mealtime routines and basic nutritional monitoring.
• Promote infection control practices and safe medication assistance if allowed by state policy.

Qualifications

Completion of a state-approved nurse aide training program and certification; high school diploma or GED preferred.

Active CNA certification in good standing per state requirements.

• CPR/BLS certification
• Infection control training

Experience in long-term care or assisted living settings; strong communication and teamwork abilities.

Experience

• Direct resident care in LTC or SNF environments.
• Experience documenting care in resident records and EHRs.
• Experience working with multidisciplinary teams to implement care plans.

Work Environment

Residential facility setting with routine scheduling; teamwork with RNs, LPNs, and care assistants; documentation in the electronic record and care plans updated regularly.

Care routines tailored to residents with chronic conditions and functional limitations.
