Impactful Family-Based Care Manager - Intensive Treatment

Eliot Community Human Services in Lynn, MA, is seeking a Care Manager for Family Based Intensive Treatment (FIT). You will work with youth and families, coordinating care across schools, clinicians, and community partners, with a focus on strengths-based, culturally responsive support. The role requires a bachelor’s degree in human services or related field, two years’ experience with the target population, and a valid driver’s license.
