Social Worker (SW)

The Social Worker is a member of the core interdisciplinary team responsible for: Performing psychosocial assessments of patients and families. Developing psychosocial outcomes to alleviate identified symptoms. Providing psychoeducation, supportive counseling, and casework interventions. Offering psychosocial education and consultation to the interdisciplinary team (IDG). Salary Range: $28 - $33 / Hour Additional responsibilities include: Participating in the preparation, evaluation, and execution of the interdisciplinary care plan to ensure high-quality psychosocial care. Conducting bio-psycho-social-spiritual assessments (not diagnostic). Providing individual and family supportive counseling. Referring patients and families to community resources. Maintaining accurate and timely documentation in the patient’s medical records. Attending weekly team meetings for care planning. Participating in patient case reviews. Providing psychosocial education and consultation to the team. Participating in supervision meetings and trainings; may supervise other team members, including interns. Coordinating volunteer interventions on the care plan. Providing bereavement care, including group activities. Participating in psychosocial spiritual on-call rotations. JOB REQUIREMENTS Ability to travel as required. Thorough knowledge or willingness to learn about community resources. Understanding of psychosocial issues in terminally ill patients and families, with knowledge of intervention strategies. Ability to work well within an interdisciplinary team. Excellent listening and communication skills. Ability to advocate effectively for patients and families within the healthcare system and community. QUALIFICATIONS Master's of Social Work from an accredited school (per 2008 Medicare Hospice Conditions of Participation, unless grandfathered in). At least one year of social work experience in healthcare. Experience with terminally ill patients and families is preferred. State licensure and/or certification as required.
