Registered Nurse - Home Health

Job Description

Job Summary:
Provides comprehensive nursing care to patients in the home setting by coordinating assessments, developing and implementing care plans, and ensuring safety and compliance with regulations. Collaborates with multidisciplinary teams to promote optimal patient outcomes and support recovery in the patient's home environment.

Responsibilities:

• Assess patients' physical, psychosocial, and environmental needs during initial and ongoing visits.
• Develop, implement, and modify individualized care plans based on patient assessments and medical directives.
• Supervise Certified Nursing Assistants (CNAs) and coordinate with other healthcare professionals to deliver quality care.
• Complete required documentation, including OASIS data, in compliance with federal, state, and agency regulations.
• Educate patients, families, and caregivers on health management, medication administration, and safety protocols.
• Maintain a safe, clean environment, adhering to error prevention and confidentiality standards.

Qualifications:

• Possess a valid Illinois State nursing license in good standing.
• Have current or willing to obtain active BLS certification.
• Demonstrate at least 2 years of Med Surg experience, with some Home Health experience preferred.
• Hold a valid driver's license, auto insurance, and have reliable transportation.
• Be vaccinated against COVID-19 and influenza or willing to provide declination.
• Possess OASIS and start-of-care experience, with strong assessment and documentation skills.
