*Responsibilities*

* Conduct thorough patient assessments, including vital signs, and medication reconciliation to develop individualized care plans.
* Monitor patient progress and respond swiftly to medical emergencies using CPR, and emergency response procedures.
* Maintain accurate medical documentation within Electronic Health Records systems, ensuring compliance with HIPAA regulations and patient confidentiality standards.
* Educate patients and families on disease management, medication administration, wound care, and health promotion strategies.
* Collaborate with physicians, therapists, and other healthcare professionals in clinical interprofessional teams to optimize patient care plans.
* Ensure compliance with all health regulations, nursing policies, State Board of Nursing standards, and Anatomy and Physiology knowledge requirements.

*Skills*

* Strong nursing background in hospital medicine, emergency medicine, geriatrics, or post-acute care environments.
* Expertise in infectious disease care, urgent care procedures, outpatient clinics, school clinics, or sub-acute care settings.
* Proficiency with EMR/EHR systems for documentation and medical records management.
* Skilled in patient assessment techniques including vital signs measurement and physiological monitoring.
* Knowledge of HIPAA regulations for maintaining patient confidentiality and handling sensitive medical information securely.
* Ability to perform injections, venipuncture, first aid procedures, CPR operations confidently.
* Familiarity with medical terminology, anatomy knowledge, medication administration protocols, and clinical interprofessional collaboration.
* Demonstrated ability to handle manual handling tasks safely while complying with health and safety standards.
* Excellent communication skills for parent education in pediatric or school settings; adept at managing patient care across diverse populations including geriatrics.
