Value-Based NP/PA Opportunity

Value-based NP/PA, Monday-Friday (no weekends or evenings) managing chronic conditions of a high-risk Medicare/Medicaid geriatric patient panel. Be part of a growing team of 20+ providers across Detroit achieving high quality-outcomes through our philosophy of 'More Time with the Patient'.

Responsibilities

The Advance Practice Providers we seek are those who have directly related experience in geriatrics, internal medicine, family practice, and primary care. The APPs who wants to practice medicine with a focus on patient care, not volume. We want our clinicians to take the time needed to truly address the patient's needs.

Qualifications

Licensed as a NP or PA in the State where HarmonyCares Medical Group office located
Federal DEA License/eligibility
Geriatric training/experience preferred
Active CPR/BLS Certification
Board Certification
Valid and unencumbered driver's license with good driving record
