Work Environment

This position is based onsite in patients' homes throughout the South River area. You will work in diverse settings that include private residences and ensure continuity of care outside traditional healthcare environments. Key Responsibilities

• Conduct thorough examinations and evaluations of patients, identifying their functional limitations and disabilities to develop tailored treatment plans.
• Implement treatment protocols based on patient evaluations and continually reassess progress towards recovery goals.
• Utilize evidence-based assessments and keep accurate documentation to track patient progress, ensuring compliance with state and federal regulations.
• Consult and collaborate with other healthcare professionals as needed to modify care plans and enhance patient outcomes.
• Provide education to patients and their families about treatment protocols and strategies for managing conditions.
• Complete all required training and maintain certification as per corporate and clinical service standards.
• Facilitate discharge planning and follow-up care, ensuring a smooth transition for patients from treatment back to everyday activities.
• Participate in team meetings, contributing insights to enhance access to care and patient engagement initiatives.

Patient Population / Client Focus

The primary patient population you will work with includes individuals experiencing a range of musculoskeletal conditions, post-operative rehabilitation needs, and chronic illnesses that affect mobility. Your expertise will aid in restoring function and enhancing the quality of life for patients of all ages in their own home environments.

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited program.
• Current state license as a Physical Therapist (PT) issued by the Physical Therapy Examining Committee.
• A minimum of two years of experience working as a Physical Therapist, preferably in a home care or outpatient setting.
• Current CPR certification.

Preferred Experience

• Experience working with orthopedic patients and knowledge of advanced rehabilitation techniques.
• Familiarity with rehabilitation documentation systems and electronic medical records (EMR).
• Proficiency in delivering patient education and developing community outreach programs.

Compensation and Benefits

The annual salary for this full-time position ranges from USD 100,000 – 120,000, commensurate with experience.
