Responsibilities:
Provide high-quality patient care in a variety of settings, including rehabilitation and long-term care
Develop and implement individualized care plans in collaboration with the interdisciplinary team
Conduct assessments, administer medications, and perform treatments as needed
Educate patients and their families on disease management, medication use, and preventive health measures
Maintain accurate and up-to-date records of patient care and treatment plans
Collaborate with other healthcare professionals to ensure seamless coordination of care
