LVN / LPN - LTC/SNF

Nursing Position in Long-Term Care

Provide compassionate nursing care in a long-term care or skilled nursing facility setting. Ensure patient well-being through specialized support and adherence to healthcare standards.

Responsibilities:

• Administer medications and treatments accurately according to physician orders.
• Monitor and document patients vital signs, symptoms, and progress regularly.
• Collaborate with the healthcare team to develop and implement patient care plans.
• Educate patients and family members on health management and post-discharge care.
• Maintain compliance with all facility policies, procedures, and regulatory requirements.

Qualifications:

• Valid LVN or LPN license in the state of practice.
• Proven experience working in LTC or SNF environments preferred.
• Ability to demonstrate strong clinical skills and attention to detail.
• Excellent communication and interpersonal skills.
• Must complete required vaccinations, including COVID-19, or hold documented exemptions.
