Position Summary
We are currently seeking a PRN Medical Social Worker (MSW) to serve patients in the Orange, Osceola, Seminole and Brevard County . This role is vital to helping patients and families navigate the emotional, social, and practical challenges that may arise during the course of treatment and recovery.

Key Responsibilities

• Conduct psychosocial assessments to determine patient needs and develop individualized care plans
• Provide counseling and emotional support to patients and families
• Assist with accessing community resources, financial assistance programs, and long-term care planning
• Collaborate with the interdisciplinary healthcare team to ensure comprehensive patient care
• Support discharge planning and coordination of services
• Maintain accurate and timely documentation in compliance with agency and regulatory requirements

Qualifications

• Master's degree in social work (MSW) from an accredited institution
• Current licensure in the state of Florida (LCSW)
• Minimum of one year of experience in a medical or home health setting
• Strong understanding of community resources and healthcare systems
• Excellent communication, documentation, and organizational skills
• Ability to work independently and manage a flexible schedule
• Compassionate, patient-centered approach
