Licensed Practical Nurse (LPN) Primary Care/ Ambulatory Care

Summary This position is eligible for the Education Debt Reduction Program (EDRP) – a student loan payment reimbursement program. Candidates must meet specific eligibility requirements per VHA policy and submit an EDRP application within four months of appointment. Program approval determines the award amount (up to $200,000) and eligibility period (one to five years) after review of the application. Former EDRP participants are ineligible to apply. Qualifications Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements United States citizenship; non‑citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA policy. English language proficiency; in accordance with 38 U.S.C. 7402(d) and 7407(d), proficiency in basic written and spoken English is required for direct patient care positions. Education: Graduate of a practical or vocational nursing school approved by the appropriate state agency or accredited by the National League for Nursing Accrediting Commission (NLNAC) at the time the program was completed. Licensure: Active, fully current, and unrestricted licensure as a Graduate Licensed Practical or Vocational Nurse (LPN/LVN) in a state, territory, or Commonwealth of the United States or the District of Columbia. GS‑03: None beyond the basic requirements. GS‑04: Six months of qualifying experience as an LPN or LVN, or graduation from an approved school with one year of experience in a nursing care environment; or graduation from an approved school of at least 24 months duration. GS‑05: Completion of at least one year of qualifying experience at the GS‑4 level or equivalent. GS‑06: Completion of at least one year of additional qualifying experience at the GS‑5 level or equivalent, fully meeting all performance requirements for the GS‑5 LPN/LVN. Knowledge and ability to provide a full range of practical nursing care to patients with a variety of physical and/or behavioral problems; ability to serve as a responsible member of the nursing team and interact appropriately with patients, families, and other support personnel. Skills in medication administration, emergency response, equipment safety, and documentation as permitted by local facility policies. Preferred experience: At least 2–3 years of professional experience working in a primary care or ambulatory care setting. Physical requirements: Extensive periods of sitting, standing, walking, and routine lifting, pushing, stooping, squatting, kneeling, and crouching. Duties Position located in the Nursing and Ambulatory Care service line at the Manchester, New Hampshire VA Medical Center. Perform assigned duties to promote quality health care, positive customer service, and a veteran‑centered environment. Work under the guidance of the Nurse Manager or designee. Serve as an LPN on a Patient‑Aligned Care Team (PACT) in Primary Care. Assist with intake of veterans, maintain clinic flow, complete clinical reminders and preventive health screenings, and document all data in the electronic medical record. Report relevant patient information to the Clinic RN or provider; prioritize and manage time effectively in the clinical setting. Assist providers with exams and procedures; perform treatments such as electrocardiograms, vital signs, weights, blood glucose readings, basic foot care, bladder scans, and ear flushes. Track and administer required immunizations; administer ordered medications and monitor for side effects. Participate in patient care management and follow‑up; initiate procedure preparations and manage therapeutic or complications arising from procedures. Respond promptly to emergency situations, including administering CPR and initiating emergency measures. Use VA computer systems efficiently and safely, adhering to facility and VA policies. Participate in PACT delivery system activities such as huddles, pre‑planning, and team meetings. Maintain professional relationships with patients, families, medical staff, volunteers, and co‑workers; observe, record, and report changes in patient condition. Assist in developing and revising patient‑driven holistic life care plans and collaborate with core and expanded teams. Protect confidentiality of patient and employee information electronically, verbally, and in writing. Work safely, following infection control protocols and equipment operation guidelines. Apply nursing research, evidence‑based practice, and literature to daily practice. Understand aging changes and principles of growth and development for adult and geriatric populations. Assess and interpret patient health data to deliver appropriate care for specific age‑related needs. Additional clinic duties as assigned. Schedule: Full‑time, Monday through Friday, 8:00 am to 4:30 pm. Schedule may be adjusted to support facility needs. Position is not virtual. Relocation and recruitment incentives: None authorized.
