Summary The Licensed Practical Nurse (LPN) will provide health services and direct care to individuals with disabilities living in a residential program. The incumbent will also provide advocacy, liaise with community health care providers, monitor medical records, and administer medication. Schedule Sun 9:00 am–5:00 pm; Mon‑Thu 9:00 am–3:00 pm; Fri 9:00 am–5:00 pm (40 hours) Essential Duties And Responsibilities Provide direct specialized nursing and other day‑to‑day care to individuals. Document health care services following appropriate state regulations, nursing standards, and Massachusetts LPN license requirements; abide by all agency policies and procedures. Function under the supervision of the residential manager in collaboration with the registered nurse and Director of Health Care. Act as a liaison to other community agencies and treatment providers regarding regular medical needs and care; help staff identify appropriate community health resources. Provide in‑service education to individuals and/or staff on health issues. Maintain knowledge of the Department of Public Health Medication Administration Program (MAP) current policies and procedures. Administer medications to assigned individuals following MAP policies and procedures. Provide quality assurance reviews of medication systems and MAP compliance. Work collaboratively with the residential manager and registered nurse by meeting weekly or as needed for medical updates. Participate in monthly nursing staff meetings and agency‑wide healthcare planning. Collaborate with the residential manager and registered nurse to assess, organize, update, and maintain health care and medication needs, including: Individual client assessment Review of routine and other physical exams Review of physician‑ordered treatments Assurance of treatment follow‑up and coordination of medical and psychiatric care Maintenance of medical charts Other duties as assigned. Direct Support Responsibilities Assist individuals with all activities of daily living, including grooming, bathing, oral hygiene, feeding, incontinent care, toileting, colostomy care, prosthetic appliances, transferring, ambulation, range of motion, communication, or other needs in keeping with the individuals’ care requirements. Check vital signs, weight, and apply creams or ointments as needed. Maintain comfort, privacy, and dignity when delivering services. Observe residents for changes in condition or behavior and report promptly to the Residential Manager. Provide services in accordance with DDS, CARF, and all applicable regulatory standards. Identify and secure appropriate medical, clinical, and financial services as needed. Transport individuals to medical, recreational, and other activities outside the home. Assist with housekeeping and cleaning duties to maintain a safe, clean living environment. Maintain progress notes per DDS and regulatory standards. Write annual assessments on individual goals per the Individual Service Plan (ISP) and participate in ISP meetings. Assist individuals in accessing community resources and recreational opportunities. Comply with Bridgewell Awake Overnight Call‑in Procedures if applicable to assigned shift. Maintain core training certifications including CPR/FA, OSHA, and PABC, and complete training in MAP. Required Education & Experience Graduation from an accredited and approved licensed practical nurse program. Current Massachusetts Nursing license. Preferred: Two years of general nursing experience and/or service to individuals with developmental disabilities in a community health setting. Preferred: Experience working with adults with DD and behaviors. Driver’s license for at least one year with a good driving record. Ability to drive a 12‑passenger van (training provided). Required Skills/Knowledge Excellent written and oral communication skills. Excellent organizational and problem‑solving skills. Ability to foster positive relationships. Basic to intermediate computer competency. Physical Demands Frequent stair climbing. Must be able to drive a vehicle. Normal eyesight and hearing, with or without corrective lenses.
