Travel Physical Therapist - SNF - $2,332 per week

Position Overview Travel Physical Therapist for a 13‑week assignment in Lancaster, California, starting ASAP. 40 hours per week on 8‑hour days. Employment type: Travel. Responsibilities Assess referred patients requiring rehabilitation services. Provide direct patient care, evaluating medical condition, functional capabilities, limitations, and potential for rehabilitation. Establish and administer a treatment program with specific goals according to patient’s capacity and tolerance under physician direction. Set realistic and achievable goals, document and record patient condition, and educate patients and families about appropriate physical therapy methods. Develop a plan of care for each patient. Provide skilled physical therapy services/interventions per physician orders. Assess patient needs, plan, evaluate, and modify care to meet goals of physical therapy interventions. Collaborate with all disciplines to plan and evaluate team goals for each patient. Qualifications Graduate from an accredited school.
