Field Nurse: Advocacy & Case Management
Are you ready to trade the clinical floor for the open road? We are seeking an experienced LPN, HHA, CHW to provide critical advocacy and health service coordination across a five-county territory. This role is designed for the independent professional who thrives on variety and self-management.
- Premier Mobility Package: Travel is a core part of this role, so we make it seamless. You will be assigned a company vehicle with gas and maintenance fully covered by the program.
- Unmatched Flexibility: Forget the 9-to-5 grind. Work a 4-day week and manage your own calendar. Whether you are providing phone consultations from home or visiting clients in the field, you call the shots.
- Professional Advocacy: Use your clinical background to navigate the intersection of healthcare and social services. You’ll be the "boots on the ground" helping families in Bucks, Lehigh, Montgomery, Northampton, and Philadelphia Counties secure the resources they deserve.
