Description

• Conduct thorough patient evaluations in both inpatient and outpatient settings to create personalized treatment plans
• Implement effective physical therapy interventions targeting mobility, strength, balance, and overall functionality
• Work collaboratively with nurses, physicians, case managers, and other rehabilitation professionals to maximize patient outcomes
• Guide and educate patients and their families on therapeutic objectives, discharge processes, and home exercise routines
• Accurately document evaluations, treatments, and patient progress following facility and regulatory standards
• Engage in care team rounds and interdisciplinary discussions to ensure seamless patient care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent credential from an accredited institution

Certifications

• Valid state licensure as a Physical Therapist
• CPR certification is preferred
• Specialized certifications (such as in neurology or geriatrics) are welcomed but not mandatory

Skills

• Prior clinical experience is highly preferred
