Registered Nurse Care Manager

Humana is seeking a Registered Nurse Care Manager to coordinate holistic, member-focused care for diverse populations. In this role, you will perform telephonic and virtual assessments, build individualized care plans, and collaborate with physicians, pharmacists, and social services to close gaps in care and improve outcomes. You’ll educate members and caregivers, support care transitions, monitor progress using clinical guidelines and data, and document accurately in electronic systems while working within a collaborative, inclusive culture focused on well-being and continuous learning.
