Schedule: Monday–Friday | 8:00 AM–4:30 PM or 8:30 AM–5:00 PM
Salary: $136,406.40–$200,000 per year
Job Overview
We are seeking an experienced Ambulatory Clinical Pharmacy Specialist to join our healthcare team in Dover, Delaware. This role provides comprehensive ambulatory clinical pharmacy services focused on Comprehensive Medication Management (CMM), chronic disease management, specialty medications, medication optimization, and improving patient access to affordable therapy .
The Clinical Pharmacy Specialist serves as a credentialed and privileged member of the healthcare team and practices under applicable Collaborative Practice Agreements (CPAs) . The Specialist works closely with physicians, prescribers, nurses, and multidisciplinary teams to develop, implement, monitor, and document safe, effective, and evidence-based medication treatment plans.

Key Responsibilities
- Provide comprehensive ambulatory clinical pharmacy services under applicable Collaborative Practice Agreements.
- Conduct patient visits focused on medication management and chronic disease management.
- Perform comprehensive medication reconciliation and identify medication-related problems.
- Assess medication effectiveness, safety, adherence, and therapeutic outcomes.
- Review laboratory results and monitor medication-related laboratory parameters.
- Develop and monitor individualized medication treatment plans.
- Provide patient and family counseling and medication education.
- Make medication recommendations and therapy adjustments as permitted under applicable CPAs.
- Collaborate with physicians, prescribers, nurses, and multidisciplinary healthcare teams.
- Respond to provider referrals and maintain closed-loop communication with referring clinicians.
- Serve as a clinical pharmacy resource for healthcare providers and staff.
- Provide clinical oversight for patients receiving specialty medications.
- Support specialty medication initiation, monitoring, and continuity of therapy.
- Assist patients with medication access and affordability.
- Coordinate prior authorizations, benefit investigations, and financial assistance programs.
- Identify and address barriers to obtaining prescribed medications.
- Support medication safety, quality improvement, and cost-reduction initiatives.
- Precept pharmacy students, interns, and/or residents as assigned.
- Participate in pharmacy education, professional development, committees, and workgroups.
- Assist with pharmacy policy development and program improvement.
- Develop and improve ambulatory and specialty pharmacy services.
- Participate in research and quality improvement activities when applicable.
- Contribute to publications and professional initiatives as appropriate.
- Maintain accurate and timely clinical documentation in the electronic health record.
- Report clinical outcomes in accordance with organizational and applicable professional standards.
Required Qualifications
Education
- Bachelor's Degree in Pharmacy required.
Licensure
- Current, unrestricted pharmacist license required.
Board Certification
Candidates must hold current Board of Pharmacy Specialties (BPS) certification in one of the following:
- Pharmacotherapy (BCPS)
- Ambulatory Care (BCACP)
- Cardiology (BCCP)
- Geriatrics (BCGP)
Experience
- Minimum of 1 year of clinical pharmacy experience .
- Pharmacy residency experience may count toward the required clinical experience.
Preferred Qualifications
- Ambulatory care pharmacy experience.
- Experience practicing under Collaborative Practice Agreements.
- Experience with Comprehensive Medication Management.
- Experience managing chronic disease states.
- Specialty pharmacy experience.
- Experience with prior authorizations and medication access programs.
- Experience with financial assistance programs and benefit investigations.
- Experience precepting pharmacy students, interns, or residents.
- Strong interdisciplinary collaboration experience.
- Experience with quality improvement, program development, or pharmacy service implementation.
