Duties and Responsibilities Provide comprehensive, high-quality primary care to adults with lived experience of homelessness in community-based settings, including permanent supportive housing, transitional shelters, safe havens, drop-in centers, and through Street Medicine. Deliver person-centered, trauma-informed, and evidence-based care to medically complex and vulnerable populations. Collaborate closely with interdisciplinary teams, including psychiatry, behavioral health clinicians, social workers, case managers, and street outreach staff, to coordinate care and address medical, behavioral health, and social needs. Participate in integrated care models through co-located clinical services and regular interdisciplinary collaboration. Maintain accurate, timely clinical documentation, utilizing protected administrative time to support quality care and compliance. Manage a balanced and sustainable patient caseload with a focus on quality outcomes rather than volume. Participate in ongoing professional development activities, including weekly protected educational time and an accredited Grand Rounds series. Engage in program development and continuous quality improvement initiatives to strengthen service delivery and client outcomes.
