Key Responsibilities

Provide occupational therapy evaluations and treatments to residents in a skilled nursing facility setting
Develop and implement individualized treatment plans to improve patients' functional independence
Document patient progress, goals, and outcomes in accordance with facility and regulatory standards
Collaborate with interdisciplinary care teams to coordinate comprehensive patient care
Educate patients and family members on therapeutic techniques and home exercise programs

Required Qualifications

Active Occupational Therapist licensure in the state of Tennessee
Graduate of an accredited Occupational Therapy program
Current NBCOT certification
Experience working in a skilled nursing facility (SNF) setting

Preferred Experience / Skills

Prior travel or contract therapy experience
Familiarity with MDS documentation and PDPM reimbursement models
