Clinical DietitianJoin Fairview as a Clinical Dietitian floating between multiple adult units at Fairview University Hospital and Fairview Riverside Hospital in Minneapolis, MN and make a meaningful impact on patients' health and well-being every day. In this role, you'll collaborate with an interdisciplinary care team to provide medical nutrition therapy, assess and manage patients' nutritional needs, develop individualized care plans, and deliver education to patients, families, and the community. You'll also contribute to the growth of future professionals by precepting dietetic interns and supporting quality improvement and research initiatives. This is a full-time, onsite position (0.8 FTE, 64 hours per pay period) with no weekends, offering an excellent opportunity to build rewarding patient relationships while maintaining work-life balance.ResponsibilitiesDevelops Patient's Nutrition Care PlanCompletes nutrition assessment according to department policy.Develops an individualized plan based on age, culture, social consideration conditions, and other relevant needs.Collaborates with patient, family and other healthcare professionals.Demonstrates critical thinking and problem-solving skills.Orders supplements, diets, nutrition therapy and other pertinent items according to the Medical Nutrition Therapy Protocol PolicyDocuments appropriately and revises as necessaryCollaborates with the interdisciplinary team for safe, quality care.Identifies interdisciplinary resources needed to assist in implementation and makes referrals as needed.Applies knowledge and principles established in the Medical Nutrition Therapy Practice Guidelines to determine the most appropriate action plan.Evaluates, systematically and continuously, the patients' response to interventionsProvides Patient/Family/Community EducationPlans and presents educational programs which meet the assessed needs and capabilities of inpatients, outpatients, community groups, and health care professionals using appropriate department educational materials and lesson plans. Considers population served.Evaluates patient knowledge, readiness to learn, and potential for behavioral changesAssists patients, families and community members in identifying and securing available resources to achieve necessary goals.Serves as a preceptor and helps clients and dietetic interns/students apply nutrition knowledge and skills.Appropriately documents in the medical record.Maintains current educational material and resources.Uses communication strategies to achieve desirable outcomes and enhanced teamworkCommunicates and receives verbal and written information in a clear, concise, accurate, and timely manner.Communicates with members of the healthcare team, patients and families and documents relevant information accordingly.Documentation follows organizational and/or regulatory policies and procedures.Communicates information to the appropriate personnel in the Nutrition DepartmentDemonstrates Quality in PracticeImplements quality practice by following system policies/procedures, practice guidelines, and Medical Nutrition Therapy protocols.Effectively uses resources as personnel, monies, equipment and time in the provision of nutritional care.Provides care according to organizational ethics, Code of Ethics for profession of dietetics, licensure, and the Academy of Nutrition and Dietetics (AND) Standards of Professional Practice (SOPP) for dietetic professionals.Meets Joint Commission and/or other regulatory standards.Identifies performance improvement criteria to monitor effectiveness of services and participates in the performance improvement process activities.Systematically evaluates quality and effectiveness of practice and revises practice to incorporate evaluation results.With specialized training, demonstrated competency, and delineated clinical privileges, may place nasoenteric access devices and bridlesRequired QualificationsB.S./B.A.Commission on Dietetic Registration/Academy of Nutrition and Dietetics (A.N.D) registered or registration eligible - if not already registered within 180 DaysMinnesota State Licensure from the Minnesota Board of Dietetics and Nutrition Practice - if licensed outside the state of Minnesota, must have receipt of licensure within 6 months of hire.
