RN, Care Manager (Home Health)

Pay rate: Weekly Gross: $2800.00 (Weekly Stipend non-taxable: $2000.00 Taxable Weekly:$800) Blended Hourly: $70.00 Shift: Days 8:00 AM to 4:30 PM 40 hours/Week Provides excellent patient care by assisting in collaboration, development, implementation, revision and reporting of the case management program. Acts as a liaison between the patient, family, nurse, physicians, multidisciplinary team and patient's healthcare benefactor to optimize outcomes. Serves as a consultant to the healthcare team on specific patient items.

Day Shift, 40 hours per week, coverage area is Lake County, CA; mileage (government rate) is in addition as travel is needed to see patients THIS IS FOR HOME HEALTH NOT HOSPITAL, Home Health experience is A MUST/REQUIRED.
