Description

Gait Mechanics is seeking per diem Physical Therapists to provide services in home in Palm Beach, Martin and Broward Counties. We offer the ability to set and manage your own schedule, with full caseload potential and competitive compensation.

Job responsibilities include

• Perform patient evaluations and establish treatment interventions in accordance with Florida Physical Therapy Practice Act.
• Accurately implement treatment interventions following physical therapy plan of care established in the evaluation.
• Complete Progress Notes and Re-assessments to monitor patient response to treatment plan every 10 visits or sooner as appropriate.
• Complete Home Safety Evaluation and offer necessary recommendations for home modification for fall prevention.
• Complete patient and caregiver training sessions in order to promote highest level of functional independence, and ensuring prevention of falls.
• Communicate with clinical team including but not limited to SLP, OT, PT and PCP.

Candidates must possess

• Liability Insurance
• Experience with dementia and Parkinson’s preferred
