Position Overview

This is a contract assignment for a Home Health position. The successful candidate will be responsible for providing home visits and attending periodic office meetings.

Contract Details

Duration: 13 weeks
Schedule: Monday to Friday, 8:00 AM - 5:00 PM
Weekend Rotation: None required
On-Call Requirements: No on-call duties
Travel Radius: Up to 75 miles from the clinic
Weekly Travel: Approximately 300 miles

Job Expectations

Productivity Expectations: Average of 7 patients per day, totaling 30 points per week.
Documentation: All charting must be completed within 24 hours of patient appointments.
Patient Acceptance: Clinicians are expected to accept all scheduled patients regardless of distance.
No Guaranteed Hours: Weekly productivity must be met to support financial needs.

Qualifications

Experience: Minimum of one full year of recent experience in adult home health or hospice.
Certifications: Current CPR and BLS certifications are required.
EMR System: Must be proficient in HomeCare HomeBase (HCHB) and have used it within the last six months.
OASIS Proficiency: Required for RNs, PTs, and OTs.
Transportation: Reliable transportation, valid driver's license, and agency-required liability insurance are mandatory.

Additional Information

This position involves driving to patients' homes and the care center for weekly meetings. Local candidates are accepted and are expected to meet all specified requirements.
