Goodwin Living in Alexandria, VA seeks an RN Clinical Liaison to act as the primary clinical representative for Home Health and Hospice. You will develop referral relationships, coordinate admissions, perform clinical assessments, and educate patients and families to ensure smooth transitions of care. You will manage a caseload focused on end-of-life care, collaborating with the interdisciplinary team to meet patient/family goals, and you may qualify for an annual team performance bonus. #J-18808-Ljbffr Goodwin Living
