Recuperative Care Case Manager I

Base Pay $31.00 - $35.00 / Hour Relocation Expense Covered No Employee Type Non-Exempt Required Degree 2 Year Degree Manage Others No JOB SUMMARY This role provides comprehensive case management, Enhanced Care Management (ECM), and care coordination services for older adults transitioning from hospitalization into Curry Senior Center’s Recuperative Care Program. The program serves medically vulnerable older adults experiencing homelessness, housing instability, behavioral health challenges, and substance use disorders who require ongoing support, stabilization, and coordinated care following hospital discharge. This role also supports patients with complex medical and psychosocial needs within a recuperative care setting located in a permanent supportive housing environment. This role works collaboratively with hospitals, discharge planners, nursing staff, ECM teams, managed care partners, and community providers to support medical stabilization, care plan implementation, housing navigation, benefit coordination, and successful transitions into permanent supportive housing. ESSENTIAL DUTIES AND RESPONSIBILITIES • Conduct screenings, assessments, intake coordination, and comprehensive case management services for Recuperative Care patients. • Coordinate hospital discharges, admissions, and transitions into the Recuperative Care Program. • Collaborate with hospital discharge planners, medical teams, ECM providers, managed care partners, and community-based organizations. • Develop and implement collaborative care and discharge planning focused on housing stabilization and long-term community support. • Support patients with benefit applications, income verification, housing applications, permanent supportive housing placement, and linkage to community resources. • Maintain an assigned caseload of approximately 6–10 Recuperative Care patients. • Participate in interdisciplinary care coordination with nursing staff, community health workers, patient care assistants, behavioral health providers, and external partners. • Monitor patient progress, barriers to discharge, and ongoing service needs. • Support patients experiencing behavioral health challenges and substance use disorders utilizing trauma-informed and harm reduction approaches. • Maintain timely, accurate, and thorough documentation in electronic medical and service record systems. • Ensure compliance with agency protocols, ECM requirements, confidentiality standards, and program documentation expectations. • Participate in required agency meetings, trainings, case conferences, and supervision. • Support a therapeutic, respectful, and professional environment within a congregate care and permanent supportive housing setting. • Respond flexibly to evolving patient acuity, operational needs, and program priorities. • Perform other duties as assigned. SCHEDULE REQUIREMENTS This position operates on a staggered schedule designed to support Recuperative Care operations, patient flow, and interdisciplinary coordination. Schedule is five days a week and will include one weekend day, either Saturday or Sunday coverage. Flexibility to support evolving operational and patient care needs is required. JOB REQUIREMENTS • Bachelor’s degree in Social Work, Psychology, Human Services, or related field preferred. • Minimum 2–3 years of case management or care coordination experience preferred. • Experience working with older adults, individuals experiencing homelessness, behavioral health challenges, and/or substance use disorders preferred. • Experience in medical respite, recuperative care, discharge planning, transitional care, post‑acute care, SNF settings, or housing‑focused programs preferred. • Experience or familiarity with Enhanced Care Management (ECM), CalAIM, managed care coordination, or complex care systems strongly preferred. • Knowledge of housing resources, permanent supportive housing systems, and community‑based services in San Francisco preferred. • Proof of negative or inactive TB required. Curry Senior Center will provide or reimburse for the cost of a TB test after an offer of employment has been accepted by both parties. Proof of a negative TB test within 90 days will also be accepted. KNOWLEDGE, SKILL, AND ABILITIES • Ability to work effectively in a fast‑paced, high‑acuity, community‑based care environment. • Willingness to work with low‑income, frail, medically complex, and culturally diverse older adults in the Tenderloin neighborhood. • Strong understanding of trauma‑informed care and harm reduction principles. • Ability to maintain professionalism, boundaries, and sound judgment in challenging situations. • Strong crisis intervention, de‑escalation, and problem‑solving skills. • Ability to build rapport and maintain supportive therapeutic relationships while maintaining accountability and structure. • Effective oral and written communications skills with the ability to express ideas in a clear, thorough, concise manner. • Strong interpersonal, communication and organizational skills. Solid aptitude for customer service. • High degree of skill in grammar, spelling and punctuation. • Ability to maintain the confidentiality of all information obtained and manage sensitive information appropriately. • Strong organizational skills and the ability to prioritize workflow. • High level of analytical problem‑solving skills. • Excellent time management skills, including the ability to manage self‑development. • Must be action‑oriented and have the ability to work independently and as a team player. • Strong documentation and computer skills, including experience with electronic medical records and systems such as Google Suite, Chrome, Apple iOS, and Microsoft Office. • Ability to adapt to evolving operational needs and patient care priorities. PHYSICAL REQUIREMENTS/DEMANDS The incumbent is in a non‑confined office-type setting in which they are free to move about at will. The Environment is mostly clean and comfortable but may include some minor annoyances such as noise, odors, drafts, etc. Work will be performed utilizing a computer, sitting at a desk/chair or standing while speaking and interacting with clients. Position will require average finger dexterity to operate normal office equipment and systems.
