RN Hospice Admission

Job Summary:

The Hospice Admission Visit Registered Nurse is responsible for admitting patients to hospice services by assessing their physical, psychosocial, environmental, safety, and developmental needs. The nurse develops individualized care plans, documents observations and interventions, and coordinates care with a multidisciplinary team. This role also involves participation in quality assessment and performance improvement programs and conducting follow-up visits as needed, primarily serving patients within San Francisco, San Mateo, and Santa Clara counties. Responsibilities:

• Admit patients to hospice services through comprehensive assessments.
• Evaluate physical, psychosocial, environmental, safety, and developmental needs of patients and families.
• Develop individualized plans of care based on assessments.
• Document observations and interventions related to the patient's environment and condition.
• Coordinate care with the interdisciplinary hospice team.
• Participate in hospice program quality assessment and performance improvement activities.
• Conduct follow-up visits as necessary to monitor patient status and care effectiveness.

Required Skills & Certifications:

• Active Registered Nurse license in the state of practice.
• Minimum of one year hospice nursing experience.
• Strong clinical assessment and care planning skills.
• Excellent communication and interpersonal skills.
• Ability to collaborate effectively with multidisciplinary teams.
• Valid driver's license, car insurance, and reliable automobile for field visits.
• Bachelor's degree in Nursing or related field.

Preferred Skills & Certifications:

• Experience with hospice quality assessment and performance improvement programs.
• Previous experience in home health or palliative care settings.

Special Considerations:

• Work primarily involves field visits across San Francisco, San Mateo, and Santa Clara counties.
• Must be able to work independently in a community/home setting.
• Night shift work required.

Scheduling:

• Night shift schedule.
• Shifts assigned based on hospice admission needs and territory coverage.
