Pay: $45.00 per hour
Schedule: 35 hours per week, Monday–Friday, with weekends as needed

Position Summary

We are seeking a dedicated and compassionate Physical Therapist to join our team at a licensed 74-bed Skilled Nursing Facility. The Physical Therapist will provide evaluation, treatment, and rehabilitation services to short-term rehabilitation patients and long-term care residents.

The ideal candidate will be committed to delivering high-quality, patient-centered care while helping residents improve their mobility, independence, strength, balance, and overall quality of life.

Key Responsibilities

• Evaluate residents’ physical abilities, mobility, strength, balance, and functional limitations.
• Develop individualized treatment plans based on each resident’s clinical needs and goals.
• Provide therapeutic exercises, gait training, balance activities, mobility training, and other appropriate interventions.
• Assist residents with improving strength, coordination, range of motion, and independence with daily activities.
• Monitor and document residents’ progress and adjust treatment plans as needed.
• Collaborate with nursing staff, occupational therapists, speech therapists, physicians, residents, and families.
• Educate residents, families, and caregivers on home exercises, mobility techniques, positioning, and safety precautions.
• Make recommendations regarding assistive devices, transfers, fall prevention, and safe ambulation.
• Participate in care planning, interdisciplinary meetings, and discharge planning.
• Maintain accurate, timely, and compliant clinical documentation.
• Follow facility policies, infection-control procedures, and applicable state and federal regulations.
• Promote a safe, respectful, and supportive environment for all residents.

Qualifications

• Doctor of Physical Therapy or equivalent degree from an accredited program.
• Current and unrestricted Physical Therapist license in the applicable state.
• Current CPR/BLS certification, or ability to obtain certification as required.
• Experience in skilled nursing, rehabilitation, long-term care, or a related healthcare setting preferred.
• Strong clinical assessment, communication, organization, and documentation skills.
• Ability to work effectively with residents, families, and an interdisciplinary care team.
• Ability to work Monday through Friday and provide weekend coverage as needed.
