East Jefferson General Hospital, part of LCMC Health, is seeking a compassionate, patient-focused Primary Care Nurse Practitioner to join our growing outpatient team. This full‑time opportunity is ideal for an advanced practice provider who is passionate about preventive medicine, chronic disease management, and building meaningful relationships with patients across the continuum of care. As a Primary Care Nurse Practitioner, you will collaborate with physicians and an interdisciplinary care team to deliver high‑quality, evidence‑based care while helping patients achieve their best health outcomes. Position Highlights Full‑time outpatient practice Collaborative physician‑led care environment Opportunity to care for patients across the lifespan Emphasis on preventive care, wellness, and chronic disease management Supportive clinical leadership and multidisciplinary team Part of the nationally recognized LCMC Health system What You'll Do Deliver Exceptional Patient Care Perform comprehensive health assessments, physical examinations, and preventive health screenings Diagnose and manage acute and chronic health conditions Develop individualized treatment plans and evaluate patient responses to care Educate patients and families on disease prevention, treatment options, and healthy lifestyle choices Respond promptly to patient calls, messages, and clinical concerns Maintain accurate and timely clinical documentation within the electronic medical record Recognize and respond appropriately to medical emergencies Identify and report signs of abuse, neglect, or other patient safety concerns in accordance with organizational policy Collaborate & Lead Work closely with physicians, nurses, clinical staff, and leadership to ensure coordinated patient care Participate in daily huddles, provider meetings, and quality improvement initiatives Serve as a clinical resource and leader for the care team Contribute to practice operations and support a culture of continuous improvement Ensure Compliance & Quality Adhere to all federal, state, and regulatory requirements Maintain patient confidentiality and HIPAA compliance Stay current with continuing education and professional development requirements Support quality, safety, infection prevention, and performance improvement initiatives Qualifications Education Master's Degree from an accredited Nurse Practitioner Program required Licensure & Certifications Current Registered Nurse (RN) license issued by the Louisiana State Board of Nursing Current Advanced Practice Registered Nurse (APRN) license issued by the Louisiana State Board of Nursing Basic Life Support (BLS) certification through the American Heart Association LCMC Health is an equal opportunity employer.
