Case Manager — Flexible Schedule with Weekend Coverage

Henry Ford Health

Henry Ford Health is looking for a Case Manager responsible for overseeing patient care coordination and discharge planning. The ideal candidate should have a Bachelor's degree in nursing or a Master's in Social Work and at least three years of clinical experience. Strong communication, problem-solving, and organizational skills are essential, along with knowledge of case management principles.
