PT - Physical Therapist

The Outpatient Physical Therapist evaluates assigned patients and develops, implements, and modifies treatment plans in accordance with the patients' needs, goals, and physician direction. This role includes the delegation and direction of all discipline-specific therapy services for assigned patients, ensuring the design of functional treatment programs for effective community reintegration. The Physical Therapist will serve as an educator to patients, families, students, and colleagues, while upholding the organization's Mission Statement and Core Values. Responsibilities Conduct comprehensive evaluations of assigned patients, utilizing clinical reasoning to develop appropriate treatment plans and goals. Maintain clear, concise, accurate, and timely medical record documentation in compliance with state, federal, and organizational policies. Ensure all care is provided under signed, valid physician orders and adheres to insurance approvals as necessary. Participate in student program assignments, fostering a positive learning environment with appropriate supervision and evaluations. Develop individualized professional growth plans, pursuing continuing education and engagement in professional activities. Adhere to state practice acts and maintain professional licensure in good standing, completing all state-mandated continuing education. Understand and implement performance improvement methodologies to enhance service quality. Required Experience / Certifications / Licensure Degree in Physical Therapy from an accredited institution. 6 months to 1+ year of outpatient clinical experience. Current CPR certification. Active and applicable state licensure for practicing Physical Therapy.
