Job Summary:

The organization is seeking a licensed Physical Therapist (PT) specialized in home health care in Valley City, North Dakota. The role involves providing physical therapy services to home health and hospice patients within a 60-mile radius around Valley City. Candidates must bring their license on the first day of employment.

Job Details:

• Location: Valley City, North Dakota


• Position: Physical Therapist (PT) Home Health


• Schedule: Monday through Friday, 8 a.m. to 5 p.m.


• Hours Per Week: 40 hours


• Required Qualifications: Must hold a current North Dakota Physical Therapy License and Basic Life Support (BLS) certification. Candidates should possess PT-related skills and have at least 1 year of relevant experience.


• Preferred Qualifications: Previous home health experience is preferred; however, it is not required.

  • Must have a current Driver's License and Auto Insurance.
  
  
  • Typical patient load ranges from 5 to 7 patients per day, experiencing conditions such as surgeries, falls, and other issues related to home health and hospice care.
  
  
  • The role requires coverage of a 60-mile radius around Valley City, ND.
  
  
  • No clinical involvement or manager interview is required.
  
  
  • No weekend or holiday work required.
  
  
  • Dress code is either CSH color scrubs or business casual.
  
  
  • The assignment duration is 13 weeks, with the possibility of hospital cancellation up to 4 shifts during this period.
  
  
  • Candidates should be aware of the radius rule (50 miles) and that they may not have been full-time, part-time, or PRN with any related facility in the past 12 months.
  
  
  • RTO (Request for Time Off) must be submitted upfront; additional RTO requests will not be approved.
  
  
  • The role utilizes the HomeCare/HomeBase electronic medical record system.
  
  
  • Upfront licensing to submit is required.
