RN Case Manager: Discharge & Care Coordination

Ochsner Health is seeking a registered nurse to facilitate multidisciplinary communications and care throughout patient hospitalization. The role requires ensuring criteria for medically necessary care and liaising among patients, caregivers, and a multidisciplinary team. Candidates must be graduates of an accredited nursing school with a current RN license and should possess strong communication and interpersonal skills.
