Certified Nursing Assistant (CNA) - Per Diem

Job Summary:

We are seeking a Certified Nursing Assistant (CNA) to join our team of dedicated healthcare professionals at Swan Lake Nursing & Rehabilitation in Patchogue, New York. As a CNA, you will play a vital role in providing high-quality care to our residents, working under the supervision of our registered nurses and licensed practical nurses. Responsibilities:

• Perform selected resident care activities, including assistance with daily living tasks, bathing, grooming, and dressing

• Assist with feeding, toileting, and ambulation as needed

• Provide vital sign monitoring, including temperature, pulse, and blood pressure

• Assist with transportation of residents between areas of the facility

• Collect and transport specimens as needed

• Respond to call bells and alarms in a timely manner

• Ensure personal alarms are in place and functioning according to individual residents' plans of care

• Communicate any changes in residents' conditions to the nurse

• Record intake, output, BM, vital signs, toileting, range of motion, ambulation, nutrition, ADL care, security device bracelets, showers/bathing, and skin checks as per facility policy

• Assist with preparing residents for examinations and treatments

• Demonstrate appropriate restraint application and release

• Maintain a clean and safe environment for residents and staff

Requirements:

• Current Certified Nursing Assistant certification in the State of New York

• High School diploma or equivalent required

• 1 year of experience as a CNA preferred

• Ability to lift, push, and pull equipment and supplies up to 50 pounds

• Ability to work in a fast-paced environment and adapt to changing situations
