Responsibilities
- Deliver occupational therapy services to adults within the skilled nursing facility, following the approved plan of care and under supervision as directed.
- Implement, monitor, and adjust treatment plans to improve independence in activities of daily living, mobility, and functional outcomes.
- Assess patient progress, document sessions and outcomes, and communicate changes to the interdisciplinary team.
- Assist with positioning, transfers, ROM, strengthening, and use of adaptive equipment as ordered; ensure patient safety and proper body mechanics.
- Support discharge planning by contributing to home exercise programs and coordination with care team members.
- Maintain infection control standards, patient privacy, and accurate charting in the electronic medical record.

Requirements
- Active Certified Occupational Therapy Assistant license in New Hampshire (or as permitted by state requirements)
- Relevant clinical experience in SNF or acute care settings is preferred
- Ability to work the 08:00–16:30 shift and meet assigned schedule
- Strong documentation, communication, and teamwork skills
- Eligibility for a 13-week travel assignment and readiness to start on or before 09/07/2026
- Commitment to delivering patient-centered, quality care and safety in a fast-paced environment
