Behavioral Health Connections (BHC) in Tempe, AZ is seeking a dynamic and talented LPN for Referral Management PRN Nights to work 12 hour shifts. The role involves assessing and coordinating the care of patients referred for Mental Health Services. Responsibilities Review assessment referrals received and collaborate with RN to assess patient needs. Assist with coordination of patient care. Current CPR, if required by a customer hospital. ("Customer hospital" means the hospital to which BHC is contracted to provide mobile assessment services.) Experience Preferred 1-3 years related work experience in Behavioral Health.
