Hospice Registered Nurse Case ManagerSalary: $80-89k/yr BOESchedule: M-F| 8:00 AM-5:00 PM +OC rotationLocation: Butler County, Ohio, and surrounding areas including Hamilton and Liberty Township, OhioRequirements of the RNCM role:Licensed Registered NurseBLS CertifiedComfortable working in both private homes and facility settings.Valid Driver's License and Auto Insurance CoverageReliable transportationA minimum of 1 year of Clinical experienceHospice experience preferredAbout the RNCM role:Conduct comprehensive patient assessments, including vital signs and medical history reviewTravel to patients' residences to conduct visitsDevelop individualized care plans in collaboration with multidisciplinary teamsCoordinate hospice care by working with other team members as well as with the patient's family members to ensure needs are met.Perform evaluations for hospice care along with informational visits as needed.Consult with the attending physician and hospice medical director concerning changes in patient condition requiring order changes and/or changes in the plan of care.Educate patients and families on treatment plans, medication management, and health maintenance strategiesSubmit complete, accurate, and relevant clinical notes promptly.Participate in IDG and case conferencesPerforms on-call duties per policy and as neededConducts an initial and ongoing comprehensive assessment of the patientsDevelop and implement the initial plan of care and work with other members of the IDG in completing and maintaining the plan of care.Supervise and evaluate the care given by the hospice Aide at a minimum of once every 14 days or as needed.Provide care plan to Hospice Aide and LPN as part of the IDG team.All other duties as assignedWhy work with Care 360 Hospice:Medical, dental, and vision health InsurancePaid Time Off401(k) MatchingLife InsuranceGrowth opportunitiesWe appreciate your interest in our Hospice Registered Nurse Case Manager.
