Mainstream Habilitation is seeking to hire a dedicated and compassionate Registered Nurse (RN) or Licensed Vocational Nurse (LVN) to join our team. Long-term care is a plus but not essential. Candidates should have strong knowledge of Excel and Word. Other job responsibilities include, but not limited to:
- Conduct thorough patient assessments including vital signs and medical history documentation
- Administer medications and injections accurately following prescribed protocols when needed
- Monitor patients' physiological status
- Collaborate with multidisciplinary teams for case management
- Educate patients and families on medication administration, health coaching, nutrition, and disease management
- Ensure infection control practices are strictly followed to prevent healthcare associated infections
- Prepare quarterly and annual assessments of client health status
Work schedule is Monday through Friday with flexible work hours if needed.
Compensation is based on experience.
