This position is fully remote, leveraging telehealth technology to reach clients in their own environments. Working from Portland, Oregon, you will engage with patients using secure video conferencing platforms, ensuring confidentiality and ease of access. Key Responsibilities\n

As a Licensed Professional Counselor at Oaks Integrated Health Centre, your responsibilities will include:

  • Conducting comprehensive and ongoing assessments to determine the psychological needs of your patients, using evidence-based techniques to develop effective treatment plans.
  • \n
  • Facilitating communication among various stakeholders—including supervisors, direct care staff, family members, and guardians—regarding patient progress and treatment adjustments.
  • \n
  • Coordinating home visits and ensuring that in-home observations, chart reviews, and staff interviews are conducted as necessary for holistic patient care.
  • \n
  • Tracking and documenting client progress through monthly and quarterly reports, maintaining accurate records to support clinical decision-making.
  • \n
  • Playing a key role in Interdisciplinary Team Meetings, contributing valuable insights and collaborating with other mental health professionals.
  • \n
  • Organizing training sessions for staff to ensure competence in the implementation of clinical intervention plans and documentation procedures.
  • \n
  • Following up on patient care through interviews and additional home visits, ensuring supportive continuity in care based on individual needs.
  • \n
  • Acting as a liaison within the mental health community to enhance program development and outreach efforts.
  • \n
Patient Population / Client Focus\n

You will work with a diverse range of clients, including adults and youth facing various mental health challenges. Your expertise will assist individuals struggling with issues such as anxiety, depression, trauma, substance misuse, and familial concerns. Providing culturally competent, empathetic support, you’ll empower clients to navigate their mental health journeys effectively.

Qualifications\n

To qualify for this role, candidates must possess the following:

  • A Master's or Doctoral degree in Counseling, Psychology, or a related field from an accredited institution.
  • \n
  • An active and unrestricted licensure as a Professional Counselor (LPC) in the state of Oregon.
  • \n
  • At least two years of professional counseling experience, with a preference for experience in clinical settings.
  • \n
  • A strong understanding of legal and ethical standards in mental health practice.
  • \n
  • Excellent interpersonal and communication skills, with the ability to connect deeply with a wide variety of clientele.

Preferred Experience\n

Specialization in areas such as family therapy, couples therapy, trauma care, or substance use treatment will be highly regarded. Familiarity with electronic medical records (EMR) systems will enhance efficiency and streamline documentation processes.

Compensation and Benefits\n
