Clinical Triage Nurse

BJC HealthCare seeks a Clinical Triage Nurse to deliver high-quality, telephone and virtual assessment for patients across its leading, mission-driven health system. In this role, you will use evidence-based protocols to evaluate symptoms, determine care urgency, and direct patients to the appropriate setting while documenting thoroughly in the EHR. Partner with physicians and interdisciplinary teams, provide clear patient education, and support community health goals. Responsibilities Provide telephone and virtual triage using evidence-based protocols to assess patient symptoms and urgency Collect and document detailed clinical histories and vital information in the electronic health record (EHR) Prioritize and direct patients to appropriate levels of care, including self-care, clinic visits, or emergency services Collaborate with physicians and interdisciplinary teams to coordinate timely follow-up and care plans Educate patients and families on symptom management, medications, and when to seek additional care Monitor and respond to incoming calls, messages, and referrals within established service standards Escalate high-risk or complex cases to providers according to clinical guidelines and organizational policies Maintain compliance with clinical quality, safety, privacy, and documentation standards Required Skills Clinical telephone triage Symptom assessment and acuity determination Use of evidence-based triage protocols (e.g., Schmitt/Thompson or similar) Electronic Health Record (EHR) documentation Patient education and counseling Care coordination and interdisciplinary communication Knowledge of urgent and emergency care criteria Clinical decision-making and critical thinking HIPAA and patient privacy compliance Time management in high-call-volume settings #J-18808-Ljbffr BJC HealthCare
