RN MDS / Care Plan Coordinator (Registered Nurse) – Full-Time

Haven of Tuscola is seeking a skilled RN MDS / Care Plan Coordinator to lead and manage resident assessments and individualized care planning. • Complete and coordinate MDS assessments (admission, quarterly, annual, and significant change)

• Develop and maintain individualized, resident-centered care plans

• Lead interdisciplinary care plan meetings and drive positive outcomes

• Ensure accurate EMR documentation and compliance with state and federal regulations

• Provide staff education and support on MDS processes and care planning best practices

• Collaborate with nursing, therapy, and administrative teams to optimize care delivery

• Active Registered Nurse (RN) license – Illinois (required)

• Minimum 1 year of MDS experience (required)

• AANAC certification (preferred)

• Strong knowledge of MDS 3.0, RAI process, and long-term care regulations

• Excellent communication, leadership, and organizational skills

• Experience with electronic medical records (EMR) systems
