RN Case ManagerPosition Summary: Coordinate patient care from admission through discharge, ensuring that treatments, services, and resources align with each patient's medical needs and recovery goals. Collaborate closely with physicians, nurses, and external providers to remove barriers to care, streamline transitions, and support safe, efficient discharge planning.Minimum Required Qualifications:TX/compact RN licenseBLS1+ year case management experience in an acute hospital settingAcute hospital experience during the past year with no more than a 90-day breakPreferred Qualifications:CCMMeditech & Careport experienceLength of Assignment: 13 weeksShift / Hours Per Week: 8A-4:30PSystems: Meditech, CareportStart Date: 10/19/2026
