Disability Services Eligibility Specialist

Care Lync

Care Lync is hiring an Eligibility Specialist/Case Manager to provide case management activities for clients with Intellectual and Developmental Disabilities in Conway, South Carolina. This role involves developing individualized care management plans and coordinating necessary services for clients. The ideal candidate will have at least 3 years of case management experience, a BS/BA degree, and proficiency in Microsoft Office.
